A leading Orange County podiatrist makes real, doctor-molded orthotics for about $525, roughly a quarter of what mall “custom fit” chains charge for an insert pulled off a shelf. Insurance usually covers most of it.

The same relief, prescribed by a doctor and molded to your actual foot, costs a fraction of that amount at Corona Foot & Ankle Group. Podiatrist Dr. Arti Amin wants to know why anyone is still paying the markup.
Here’s how that dubious scenario plays out at malls everywhere, costing unsuspecting consumers money.
Step 1: You place your foot on a glowing floor scanner.
Step 2: The screen flashes a single word: custom.
Step 3: A salesperson disappears, returning two minutes later holding a box.
Step 4: Inside is a pre-made plastic insert.
Step 5: You realize it’s one of a few stock sizes pulled off a shelf in the back.
Note: Step 5 almost never happens.
Dr. Amin knows why.
“It comes down to a play on words: ‘Custom fit.’ They fit you to a size that was already sitting on a shelf before you came in. That’s not custom anything. That’s a shoe size with a four-figure markup.”
The distinction matters more than the theater lets on.
The American Podiatric Medical Association draws a clean line: custom orthotics are prescription devices. Pre-packaged, mass-produced arch supports are simply shoe inserts; the same category sold at the grocery store and the mall. Those are often covered by insurance; retail inserts are not.
GoodRx puts a genuine doctor-made custom orthotic at $300 to $800, right where Corona Foot & Ankle’s price falls. By GoodRx’s own reporting, a multi-pair “personalized” system from a retail chain can run past $1,000. Published consumer-complaint records go higher, documenting packages billed at $1,700 and $2,000.
One shopper’s receipt topped $2,800.
More alarmingly, several chains market these inserts as non-medical in their own published materials. Then they turn around to call the very same products “medical-grade” at the register.
That second label is the reason buyers are given when the box turns out to be non-returnable.
“A real evaluation looks nothing like two minutes on a scanner,” says Dr. Amin. Before she writes a prescription, she runs a gait analysis, a biomechanical exam, in-office X-rays, and a lifestyle assessment, then scans the foot in 3D.
Even with all that technology, the orthotic still runs $525 cash, with a six-month adjustment warranty. For most PPO patients, insurance covers roughly 80 percent after the deductible.
The math is hard to argue with.
“That’s a better product for nearly $1,500 less,” Dr. Amin says. “With a doctor evaluating you.”
For Dr. Amin, the inserts are almost beside the point. What she’s defending is mobility. She has reversed surgery recommendations for patients who were never told orthotics were an option, pairing devices with strength training to correct the foot’s structure without a scalpel. The aim is keeping people walking and chasing grandkids, for their bodies and their minds alike.
“If you tell me you can’t exercise anymore, I take that very personally,” Dr. Amin says. “My goal is to get my patients back to the activities they enjoy.”
Before the next screen lights up under your feet and a salesperson promises custom, ask this valid question: Who’s the doctor in the room?

Dr. Arti Amin is a board-certified podiatrist at Corona Foot & Ankle Group. She holds the degree D.P.M. and is board certified by the American Board of Multiple Specialties in Podiatry (FABMSP) in the treatment and prevention of diabetic foot ulcers, primary podiatric medicine, and foot and ankle surgery.
She is also a Fellow of the American College of Podiatric Medicine (FACPM).
